So that we can spend more time together during your consultation, and less time with the preliminary paperwork on the day of your appointment, please download our new patient forms below. We ask that you complete the forms prior to your first visit at our office. You can mail, drop off, or fax them to (407) 682-7180.  In addition, please bring any medications, supplements, vitamins and/or herbals you are currently taking with you to your appointment. In the event your are unable to keep the recently set appointment, please call us at (407) 682-7111 so that we might reschedule a date and time that is more suitable for you.
